The hazmat endorsement costs $42.50. You'll also need a TSA background check. Schedule that at a TSA enrollment center in Warwick. Bring your passport or birth certificate.
After passing the written test, you'll get a temporary endorsement. Your actual card arrives by mail in 2 weeks.
