You can take the Indiana hazmat knowledge test at any BMV license branch that offers CDL testing. Check the BMV website for branches in your area – Indianapolis, Fort Wayne, Evansville, and South Bend all have testing. You'll need an appointment and a valid Indiana CDL permit.

The test costs $30 (subject to change). Pay at the branch. You also need a TSA background check before you take the test. Get that done online first. The BMV won't let you test without it.

After you pass, add the H endorsement to your CDL. The BMV mails your new license within 10 business days.
